1 If the member is unable to present a current blue membership card or other valid proof of membership, they must sign the waiver form.
2 Minor waivers must be signed by Parent or Legal guardian. A copy of the Parent/ Legal guardian's state ID must accompany the waiver, unless it is notarized. The Parent / Legal guardian's signature and name must be clearly visible on the copy.
